I like to have Take Home Reading and Math Homework Bags for my students. The program has been very successful and I have the support of the PTO to help replace lost materials. I also have a parent volunteer come in each week to check in the bags. If a bag isn't returned the student can't get a new one.
Most of my homework bag packets came from:
The others I developed simply based on what books I had or what I thought would be fun! Click on the title to get my document!

I hope you can get some great ideas to get your homework bags up and running. My students love them. To keep them organized I give each bag a number and have an excel spreadsheet with all of the student's names. Across the top I put the date for each check out and the bag number goes by the student's name. When the parent volunteer comes in she simply X's over the number if it is returned. This system has worked great for me and is very easy! In the beginning of the year I assign the bags to the students, especially if they need extra practice in one of the academic areas that a bag covers. By January, they get to choose their own or opt out of taking one. I make it optional because some families are just too plain busy!!
Here is my note that I send home going over the rules!
This is my reminder note... I can't remember where I got it. If it was from you, thanks and comment below so I can give you credit, please!
Good Luck! When I'm back in the classroom I'll put up a few pictures, too.
